Hi Dr. Halevi,
Can you recommend a basic acupuncture treatment for diabetes? I know that liu wei dihuang wan is used as a basic prescription for this purpose. Is there a logic in constructing such a point prescription in acupuncture?

In fact there is a standard point formula that is somewhat in parallel to the famous Six Ingredients pill. Diabetes is basically a Yin Xu (deficiency) syndrome, giving rise to apparent Yang or relative Yang. Its sprouting is from the kidneys energetic system, and it causes false heat in either the kidneys, lungs or stomach. Thus, kidney Yin deficiency manifests as excessive and frequent urination due to K Qi deficiency. The massive loss of fluids drains the lungs which are the metal mother of the kidneys thereby causes excessive thirst and dry mouth, so typical of diabetes. This drainage of the lungs has an impact upon the spleen-earth, the mother of the lungs-metal. The depleted spleen has the consequence of the thinning of the muscles and the flesh which are nourished by the spleen. Thereby the poetic name of thirsting and wasting for diabetes in Chinese. The Yin (and Qi) deficiency of the spleen affects its Yang organ, the stomach, and the stomach gets dried up. This gives rise to the excessive hunger also typical of diabetes, and so on.
To correct this imbalance, we use this formula:
Lu-5/ K-2/ Sp-6/ Ren-4/ Ren-6

Lu-5 and K-2 drain empty fire from the lung and kidney systems and moistens them. Sp-6 tonifies the spleen and blood. Ren-4 tonifies Yin Qi of the kidney and Ren-6 the Yang Qi in order to help Yang create Yin.
Other points may be also introduced to this prescription, according to specific symptoms. I.e.: Ren-12/ St-44 in excessive st. heat, or St-36 in tonification to supplement the Yin.
Liv-2/ Liv-3 when liver symptoms co-exist, such as depression, chest oppression, etc.
Ren-23 or/and Yuye to relieve excessively dried mouth and so forth.

I have read that the following combination gives good results for diabetes. I am planning to use them. Would you kindly comment on logic?

Tone Sp 3, Liv 3 and K 2 and
Sedate Sp 8, Sp 9, Liv 6 and Pericardium 3

There are many point combinations effective for the treatment of diabetes. The combination that I suggested above is usually applied when there is marked Yin deficiency which is the basic pattern involved in diabetes. Other combinations, such as the one you mention, have been grouped under specific manifestations of pulse quality, usually corresponding with the 5-phase approach. In such a case, the points have been selected to accommodate a very specific pulse pattern that may be characteristic of one patient but may be wrong for another one. It is imperative to tailor the prescription in accord with the symptoms and signs of a specific person, especially if the prescription has been devised by the 5-phase logic. The prescription which I have presented above, serves only as a guideline and not as a recommendation in every case of diabetes.
