Dear Dr. Halevi,
I understand that Chopstick Acupuncture treatment can be used by a lay person and is akin to stimulating acupoints with a probe.

Request you to suggest minimum acupoints to be toned or sedated for habitual miscarriage (three miscarriages within one and a half months of conceiving).

How toning TW 6, TW 10 and GV 16 and sedating TW 2 work on this condition?

Usually, acupuncture is very effective for habitual miscarriage. I don't recall a single failure in treatment for this problem during my years of practice. Yet, I have never tried to apply chopsticks acupuncture for this problem, so I can't guarrantee anything here. I would suggest using these points as a foundation formula:
B-23/ B-20/ Sp-6/ St-36

If you have a Xu/cold condition, use also moxa on all of these points. If you have blood deficiency, use B-17 as well. If the patient has weak lungs and/or immune system, add: B-13/ B-43. If you detect Liver deficiency, add Liv-8 and B-18... and so on. The chopstick should be the long one, used with a tonifying maneuvering, as described in the book.

I am not qualified to use needles. So I shall try alternative tonification method. Immune system also needs toning as attack of virus has overstimulated pitutory. Pitutary hormone Proactin is high needing sedation of pitutory as well. Liver is deficient. Thank you for covering aspects not described by me. I will appreciate if you guide me on preferred sequence of tonification of foundation formula. I would also benefit from your brief discussion on choice of the foundation formula acupoints. Should these points be administered now or after confirming pregnancy? What about Sp 6 after pregnancy? Any harm indicated? Any comments on Tripple warmer and Du points in my earlier query?

Choosing points that might have influence over the pituitary gland, or the thyroid, will not affect habitual miscarriage. Chinese medicine can not be applied following a western diagnosis, it doesn't work this way.
Sp-6 may be stimulated very lightly and gently even during the pregnancy, depending on the diagnosis and the point selection that it dictates. I susally treat the patient for a month or so before she gets pregnant, and then occasionally during the pregnancy.

Dear Dr. Halevi,
Thanks for guidance. My take home is:
1 Treatment should precede planned pregnancy by about a month and occasional reinforcement during pregnancy
2 Sp6 needs care, but can be used softly during pregnancy.
3 TCM should better not be applied following western diagnosis.

Now I shall use fundamental formula with immunity and liver and also study the reasons for choice in your database. I shall come back if I have difficulty in understanding. God willing if there is result, I shall come back to you after completion of three months of pregnancy and share with you. Fortunately the patient is open to non-invasive external treatment.

Hi Shmuel re Habitual Miscarriage. Do you mean threatened miscarriage? If
it were, or even for a woman who had a history of miscarriage I, for a start
would have to ask why?
I would be looking at her pulses for any signs of Ki deficiency at least.
I would take the "fix the pulses approach", but if its only 3 months to go
and things have started happening, then you would have to move a bit
quicker!
If something, ie a baby is about to "fall out" then Central Qi is not
holding it in.
I would (and have)use St.36, CV6(shallow,like 1 to 2 mm) and CV 12 again
with a similar shallow insert.
I would moxa these points with smokeless stick moxa after checking whether
the client was comfortable about moxa smoke. I have extractor fans in my
clinic
I would also use Ki3, moxa needle again as a Qi building point
I would not use Sp6 as it's a pregnancy terminating point

However as a long term point while doing constitutional work if the Client
was not pregnant, I certainly would use it
I think that at the time of her life when a woman wants to conceive she
should have all the right things happening for her Blood and Qi should be
strong and full.

If the Client is 40+ years old, then then she is approaching the sixth of
her seven year life spans when things can get a bit "dodgy" on the
conception and keeping it in front.

This may add some food for thought....Phil Macqueen

Re my last post...I would add that I am a practitioner with nearly thirty
years experience.
I would not recommend anyone needling a pregnant womans' abdomen unless they
had lots of experience
Dr. Shmuel would probably concur.
I have outlined a professional approach, not one to be taken lightly or by
one with no or minimal experience
Please be aware that life is precious.

My regards Phil, robust experience speaks volumes in a short note.

By habitual miscarriage I meant natural abortion - termination of pregnancy - foetus falling off on its own repeatedly - in the case under discussion three times within two years and a half or so - within 40 days of confirmation of fertilization of ovam thro' clinical test.

The lady is born in 1980 and not approaching "over the hill" age.

She has no problem about fertility as she has successfully conceived three times but her system could not hold the baby longer than 40 days three times in succession. Hat trick of natural abortion.

The lady has not yet conceived at the time when these guidance is sought. Therefore her system has to be made capable to hold baby thro' out nine months even before she conceives. Dr Shmuel guided that the treatment should precede conceiving at least by a month.

I am also skeptical on use of Sp 6 and several other points after onset of pregnancy and hence you and Dr. Smuel appear to me on the same page on caution, language differing marginally.

What I meant by three months is that if and when she conceives, and retains conception for three months (earlier being not more than 40 days) I shall revert back so that further guidance can be helpful.

I shall not use needles. Other methods may be less effective but fortunately available for tonifying or sedating.

Threatened abortion in early pregnancy

To treat this complication, an obstetrician prescribes hormones (oestrogens alone or in combination with pure gestagens up to the 12th week, and only pure gestagens after), in spite of the fact that hormones are undesirable during embryogenesis. Besides hormones, spasmolytics and sedatives are prescribed.
The condition of threatened abortion in TCM is called Tetal Restlessness" and is differentiated as follows

i. Kidney deficiency: pain or soreness of the waist and legs, fetal restlessness, weakness of the limbs and body, dizziness and tinnitus, abdominal pain and vaginal bleeding after a drooping sensation in the hypogastrium, Tongue: pale with thin white coating Pulse: deep, thready, slippery.

ii. Liver Depression: restless fetus or vaginal bleeding, mental depressioa distention and pain in the hypochon-drium, belching, anorexia, or acid regurgitation and vomiting of bitter fluid. Tongue: thin sticky coating Pulse: wiry and slippery.

iii. Spleen Deficiency: increased sensation of bearing down after the fetus has been disturbed, lumbago, abdominal distention, watery stools and/or abdominal pain with severe vomiting and diarrhoea Tongue: pale with a thin white sticky coating Pulse: thready and slippery

iv. External trauma.

v. Food poisoning.

vi. Deficiency Cold: fetal restlessness, oedema, vomiting, nausea, preference for heat or cold, pain in the abdomen. Pulse: deep and thready.

vii. Heat syndrome: fetal restlessness, occasional abdominal pain, subjective sensation of heat all over the body, red lips, or thirst and dark urine. Pulse: slippery and rapid.
In my routine work I have more often observed the symptoms of threatened abortion of the Kidney deficiency type, and more rarely the heat syndrome type. In the case of Kidney deficiency, the patient complains of heaviness and pain in the lower abdomen which radiates to the back and sometimes bloody vaginal discharge. The patient seldom urinates and has a frightened facial expression. The pulse is weak and the tongue pale with a slight whitish coat. The area of the Kidney on the ear is markedly white.

Treatment

The treatment principle is to tonify the Kidney. Primary points:
Taixi KID-3 - to tonify Kidney
Fuliu KID-7 - to tonify Kidney

Secondary points:
Neiguan P-6 - to calm the spirit
Tongli HE-5 - to benefit Heart Qi
Qihai REN-6 - to normalise the Qi. At this point needles should be turned only anti-clockwise without causing strong sensation.
Auricular points:
Kidney, Endocrine Glands, Ovaries, Shenmen, Heart
In obese women, threatened abortion in the early stages of pregnancy is not uncommonly caused by stagnant Qi manifesting as heat syndrome. The main symptoms in these patients may be slight uterine bleeding, slight pains or heavy sensation in the lower abdomen, or pressure on the bladder. The patient may complain of accelerated heart beat, insomnia, chills and constipation. Such patients have warm reddish faces and hands, the oral mucous membrane is dry, little saliva is produced, the tongue is red and the pulse is tense, rapid and slippery.

Treatment
In this case it is necessary to harmonize the Blood and Qi Primary points:
Taichong LIV-3 - to facilitate Qi
Xingjian LIV-2 - to benefit Liver Qi
Sanyinjiao SP-6 - to facilitate Qi and Blood in the lower part of the body, to benefit the Spleen
Xuehai SP-10 - to benefit the Spleen
Zhongwan REN-12 - to facilitate Qi in the middle burner and Spleen Qi

Secondary points:
Zhaohai KID-6 - to facilitate Kidney Qi, to harmonize the upper and lower parts of the body together with Lieque LU-7 - to facilitate Lung Qi

Auricular points:
Endocrine Glands, Liver, Ovaries, Brain, Occiput

I am grateful, Mr/Ms Annonymous,
I have noted down your guidance for threatened miscarriage - also termed as foetal restlessness. These hints will be useful when the young lady conceives and then in case there is a threat to pregnancy.
Presently, having known her past history of three miscarriages, the project is to prepare her system before conception itself so that recurrence can be prevented and there is no need for treating for foetal restlessness.

About patients receiving western treatment, the difficulty is that all who lean to alternative therapy tire themselves with western treatment and while leaning on TCM also they continue the allopathic treatment.

Dr. Halevi and other contributors, kindly guide me, how to confirm if fallopian tubes are blocked or not without leproscopy? What are the points for opening blocked fallopian tube/s? Any harm administering points for opening fallopian tube/s without confirmation of blockage?

Will the following protocol help?

TW 3 rent, GB 38 (tone), GB 44, 43 sedate.

Jan, this article is nice and presents a framework of diagnosis for a threatened miscarriage condition, or restless foetus. Yet, in practice, one usually finds either all of the listed syndrome symptoms in one patient, or none of them... and there IS still an abortion ! Many of the points listed in this article, have a strong blood quickening qualities, and I would hesitate prescribing them for a patient who faces a danger of miscarriage.

Haya: If the fallopian tubes are blocked, there is no conception. I gather that you refer to a condition wherein a lady can not conceive. I humbly suggest to use Western medicine in order to reaffirm this diagnosis properly, especially if by Chinese medicine you have reached such a conclusion, or suspicion.
If a fallopian tube is indeed clogged, you need to use in dispersion the following points:
Sp-6/ Sp-9/ St-28/29/30 - as main points
B-22/ B-39/ - secondary
L-7/ K-6 - extra meridian Ren Mai.

Re the issue of fallopian tube blockage. There would be pain if the ovum happens to go down the blocked tube during the ovulation part of the cycle and the fallopian tubes are blocked. When ovum is deposited in the abdomen there is no pain. The allopathic treatment is to shoot a burst of gas into the tube and it can be after this treatment the woman sometimes falls pregnant.

Shmuel, thank you for clarifying my previous post. I also had my doubts when I posted but still believed it valuable because it had that interesting differential diagnosis and useful to consider as prophylaxis. I have in the past treated sterility with wonderful success and assisted with many traumaless pregnancies but I have not used acupuncture for a number of years.

I do not wish in anyway to detract in anything that you say because I see the value and agree 100% with your suggestions.

My experience tells me the treatment in the areas of the Ren Mai, Kidney Jing. Psychological issues: Circulation-Sex and SanJiao (eustacian tubes and fallopian tubes should be considered as polar opposites) and intercurrent treatments should be considered using such paradigms.

I have always relate my treatments to the pulse and 5 elements.

eg. TW3 Gb38,44,43 - I love your thinking there but 8 needles?? read the wonderful article on point combining that Shmuel has on this site. I would use a one above and two below in a formula such as you suggest. Fewer needles the better and the laws of point combination are to be treasured. One of my teachers trained with wu wei ping who was a master of point combination.

Even if you do not use needles using few points when treatiing "from a distance" is better. As a supplementary treatment to specific local points.
